In furtherance of this purpose, women are encouraged to share their personal stories and comments, in accordance with the following terms: 1. Hologic, Inc. and its affiliated companies and agents (collectively, "Hologic") may use, copy, publish, and distribute my name, likeness, photo, and any information provided by me (the "personal information") in connection with its communications activities (including, but not limited to, all sales, marketing, and advertising activities) in whatever media it chooses (including, but not limited to, written, verbal, electronic, and video). 2. I will share my relevant personal experiences truthfully and professionally and will not purport to give medical advice, nor offer to sell or buy any product or service. 3. I will not post threatening, harassing, defamatory, or libelous material or intentionally make false or misleading statements or post any personal information that I know is confidential or proprietary to another. 4. I will only post material that belongs to me and does not infringe on any copyright law. 5. I am authorized to reveal all information that I share and have obtained all necessary consents and authorizations required by law; including, but not limited to, those required under the Health Insurance Portability and Accountability Act of 1996, as codified at 42 U.S.C. §1320d through d-8, as amended from time to time ("HIPAA”). 6.
